The Canadian Mental Health Association, Peel Dufferin Branch is recruiting for a Community Support Worker to provide case management support, for a maximum of 3 years to adults (16 years old to 45 years) who have experienced a first episode of psychosis as they work towards recovery.

 The FACT PEEL + Program supports the recovery of young adults experiencing mental health and addictions issues and their families. There is a primary focus on individuals who are experiencing psychosis. Service is provided in partnership with the client, family and relevant community agencies.

As the primary Community Support Worker , the incumbent will acknowledge the value of personal lived experience as a demonstration of hope, recovery and ongoing personal growth and transformation in your work supporting your clients

This is a full time permanent position (37.5 hours per week) and will be based at 7700 Hurontario St location. The incumbent reports to the Manager –Youth Services Program.


Key Responsibilities:


  1. Support an assigned caseload of clients by establishing a supportive relationship with clients and their support systems.

  2. Developing comprehensive assessments which include developing care plans, strategies to monitor care plan and assessment of when to discharge including transition planning.

  3. Identifying strengths and needs as well as developing a crisis plan to support and maintain client wellness.

  4. Orient clients to recovery and talk about ways to support recovery; by disclosing personal lived experience, inspire hope, resiliency and personal responsibility.

  5. In relationship with clients, support identification of strengths that people can access to address areas they want to change.

  6. Support clients to make choices that promote safety, wellness and recovery from Intake through discharge.

  7. Invite, encourage, and support self-advocacy of clients in terms of their clinical relationships.

  8. Facilitate WRAP and Pathways to Recovery groups and participate in the Peer Community of Support Enhance the FACT Peel + team’s understanding of how to support recovery

  9. Establishing and implementing Recovery Plans to guide the mutual work towards client recovery.

  10. Encouraging clients to identify and work towards educational and vocational goals.

  11. Providing on-going supportive counselling, life skills coaching, pre-employment coaching and appropriate referrals to enable clients to achieve their goals.

  12. Consulting, advocating and liaising with community resources and agencies to assist clients in achieving their goals.

  13. Maintaining appropriate record-keeping/documentation and client files, as required by CMHA policies and procedures.

  14. Working effectively and collaboratively with program teams, internal and external to CMHA Peel Dufferin.


Skills and Qualifications:


  1. College diploma or University Degree in Social work or a human services field.

  2. Must have lived experience in the Mental Health System. 

  3. Minimum 2 years of experience in Community Case Management.

  4. Completion of a Peer Support Recovery Education Program is preferred.

  5. Knowledge of Early Intervention Ontario Standards of care is an asset.

  6. WRAP (Wellness Action Recovery Plan) Facilitator Certification is an asset.

  7. Proficient computer skills including MS Office, Outlook, Client Record Management Systems, OCAN assessment tool.

  8. Solid working knowledge of recovery principles including working with families.

  9. Strong interpersonal and communication skills.   

  10. Strong ability to work independently and on a team.

  11. Knowledge of community services and supports in Region of Peel.

  12. Enthusiastic problem solver, grounded in positive conflict resolution and goal setting practice

  13. Knowledge of French or any other languages reflecting the diversity of Peel Region is an asset.

  14. A valid Canadian driver’s license, access to a reliable vehicle and relevant insurance. A satisfactory Vulnerable Sector Screening (Police Check) report.

  15. Current CPR/First Aid certification.

  16. Ability to work flexible hours including some evenings.
